where do I get my shot records?

So I need my shot records for the CNA and nursing program at school… I grew up in louisiana and lived there until 2004. I called my doctor and they told me they no longer had me on file since its been so long since Ive been there. So basically where else can I get these shot records? My parents are no help and Ive been searching online with no luck. HELP please
